How to communicate your wishes

For detailed guidance, please consult our Advance Directives Planning Guide, which is available for free at advance directives templates (4.1.2).  It is very important that your mandated person is appointed in writing and that you communicate to them and others what you consider to be quality of life and what your end-of-life choices would be. Ideally, your wishes are documented in a template like a living will and a template for this is available to our members (who sign up for free) via our members’ portal (6).

It is important to note that your wishes are context-specific and can change over time and in different circumstances. For this reason, advance directives, like living wills, should not be regarded as static documents but as a part of an interactive process of advance care planning and conversations that you engage in with your loved ones and healthcare practitioners.
